Mandatory Form due May 1st. This form must have completed by a doctor  within 1 year of your check-in date.  This is different than the Camper Health History form which is completed online. Please upload this form in the registration portal.

Camper with food allergies or dietary issues should contact the food service manager at least two week in advance to ensure there is a plan in place to accommodate the campers needs.
